These Terms & Conditions (“Terms”) govern your use of the Dry Eye Symptom Check tool (the “Quiz”) and any related services provided by Comprehensive Eyecare (“we,” “us,” “our,” or the “Company”), located at Suite 42, Winchester Business Centre, 15 Hope Road, Kingston, Jamaica. By completing the Quiz and submitting your information, you agree to these Terms. If you do not agree, please do not use the Quiz or submit your information.
The Quiz is a self-assessment screening tool only. It is modeled loosely on symptom-scoring approaches used by eye care professionals, but it does not diagnose any medical condition, and your results are not a substitute for an examination, diagnosis, or treatment plan from a qualified eye care professional. Your score and the accompanying description are general in nature and do not account for your full medical history.
You should not rely on your Quiz results to make any decision about your health. If you are experiencing eye pain, sudden vision changes, or any symptom that concerns you, please contact us or seek appropriate medical attention promptly rather than waiting on a Quiz result.
The Quiz is intended for individuals aged 18 or older. If you are under 18, please have a parent or guardian complete the Quiz on your behalf or contact our office directly.
To view your Quiz results, we ask you to provide your full name, email address, and phone number, and to confirm your agreement to these Terms. By submitting this information, you confirm that:
Information submitted through the Quiz (your name, email, phone number, and your Quiz score/result) is stored in our internal records for the following purposes:
We do not sell your personal information to third parties. We may share your information with staff and service providers who help us operate our practice (for example, appointment scheduling or IT support), and who are required to keep it confidential. We may also disclose information where required by law.
This collection and use of your personal data is carried out in accordance with the Data Protection Act, 2020 of Jamaica. You have rights under that Act regarding your personal data, including the right to request access to, correction of, or deletion of the information we hold about you. To exercise these rights, contact us using the details in Section 10.
We retain your information for as long as reasonably necessary for the purposes described above, or as required by law.
While we take reasonable steps to protect the information you submit, no method of electronic transmission or storage is completely secure. We cannot guarantee absolute security of information sent to us over the internet, and you provide your information at your own risk.
The Quiz score is calculated automatically based on the answers you provide. We do not independently verify your answers, and the accuracy of your result depends entirely on the accuracy and honesty of the information you enter. Technical issues (including but not limited to connectivity problems) may occasionally prevent your responses or results from being recorded or transmitted correctly; we are not responsible for any loss arising from such technical issues.
To the fullest extent permitted by law, Comprehensive Eyecare, its owners, staff, and affiliates shall not be liable for any direct, indirect, incidental, or consequential loss or damage arising out of or in connection with your use of the Quiz, your reliance on your Quiz results, or any decision you make (or fail to make) regarding your health based on those results.
Nothing in these Terms is intended to exclude or limit liability that cannot lawfully be excluded or limited under the laws of Jamaica.
The Quiz, including its design, questions, scoring methodology, and content, is the property of Comprehensive Eyecare and may not be copied, reproduced, or distributed without our prior written consent.
We may update these Terms from time to time. The “Last updated” date at the top of this page reflects the most recent revision. Your continued use of the Quiz after changes are posted constitutes your acceptance of the updated Terms.
These Terms are governed by the laws of Jamaica. Any disputes arising from these Terms or your use of the Quiz shall be subject to the exclusive jurisdiction of the courts of Jamaica.
